Before Your First Appointment
Please click on the Patient Forms link above, download, fill out, sign, and then text the forms at least 24 hours before your initial appointment.
In addition please include the following:
- Patient’s insurance card
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about your medical and surgical history
- Recent test results, x-rays, or relevant records

Insurance and Payment Information
Ghadeer Okayli, M.D. is not a provider for any health insurance plans. Client are seen on a pay for service basis. Our office can provide you with the necessary paperwork to file to your insurance carrier upon request. Questions regarding insurance coverage and benefits need to be directed to your insurance carrier.
Privacy Practices
Our Notice of Privacy Practices describes how medical information about you may be used and disclosed, and how you can get access to this information. It is given to all new patients and a copy is available upon request.

Online Evaluations
Our practice is 100% virtual.
International Evaluations
Dr. Okayli is fluent in English and Arabic. She is available for virtual international evaluations through FaceTime or WhatsApp. Please call or text our office for more details at 512-632-6003.
